Fibromyalgia and Chronic Pain are the most common cause of serious disability Fibromyalgia and Chronic Fatigue Syndrome (CFS) share a number of symptoms and the names are often used interchangeably with each other. People with fibromyalgia have "tender points" on the body; these are specific places on the neck, shoulders, back, hips, arms, and legs and these points hurt when pressure is put on them. So much is still unknown about what causes fibromyalgia, there is no go-to treatment.

Track 1: Chronic pain

Chronic pain is often defined as any pain lasting more than 12 weeks. Whereas acute pain is a normal sensation that alerts us to possible injury, chronic pain is very different. Chronic pain persists often for months or even longer.

Track 5 : Root causes

The cause of fibromyalgia is unknown. However, several hypotheses have been developed including "central sensitization". This theory proposes that people with fibromyalgia have a lower threshold for pain because of increased reactivity of pain-sensitive nerve cells in the spinal cord or brain.

Fibromyalgia is a syndrome with multiple symptoms that commonly occur together, including widespread pain, decreased pain threshold or tender points, incapacitating fatigue, and anxiety or depression. Most laboratory tests are not very useful by themselves for diagnosing fibromyalgia.

Genetic factors can explain a significant amount of the variability in the perception of pain. Fibromyalgia syndrome and related conditions are syndromes characterized by generalized pain sensitivity as well as a constellation of other symptoms. Family studies show a strong familial aggregation of FMS and related conditions, suggesting the importance of genetic factors in the development of these conditions.

Advances in understanding of the growing number of pathophysiologic mechanisms that underlie the generation of pain and the influence of the brain on the experience of pain led to the investigation of numerous compounds for treating pain. Improved knowledge of the subjective nature of pain, the variations in the measurement of pain, the mind-body placebo effect and the impact of differences in the conduct of a clinical trial on the outcome have changed approaches to design and implement studies. Careful consideration of how these concepts affect the choice of study population, the randomization and blinding process, the measurement and collection of data, and the analysis and interpretation of results should improve the quality of clinical trials for potential pain therapies.

Fibromyalgia

Fibromyalgia (FM) is a medical condition characterised by chronic widespread pain and a heightened and painful response to pressure. Symptoms other than pain may occur, leading to the use of the term fibromyalgia syndrome (FMS). Other symptoms include feeling tired to a degree that normal activities are affected, sleep disturbance, and joint stiffness. Some people also report difficulty with swallowing, bowel and bladder abnormalities, numbness and tingling, and cognitive dysfunction. Fibromyalgia is frequently associated with psychiatric conditions such as depression and anxiety and stress-related disorders such as posttraumatic stress disorder. Not all people with fibromyalgia experience all associated symptoms.

Fibromyalgia is a central nervous system (CNS) illness, which includes the brain and spinal cord.  Nerve cell endings, or receptors, are at the front end of pain sensation

Chronic pain

There are about 500 Million plus people suffer from chronic pain, defined as pain that lasts longer than six months. Chronic pain can be mild or excruciating, episodic or continuous, merely inconvenient or totally incapacitating. With chronic pain, signals of pain remain active in the nervous system for months or even years. This can take both a physical and emotional toll on a person.

The most common sources of pain stem from headaches, joint pain, pain from injury, and backaches. Other kinds of chronic pain include tendinitis, sinus pain, carpal tunnel syndrome, and pain affecting specific parts of the body, such as the shoulders, pelvis, and neck. Generalized muscle or nerve pain can also develop into a chronic condition.

Neuroscience

Its exact cause is unknown but is believed to involve psychological, genetic, neurobiological and environmental factors. There is evidence that environmental factors and certain genes increase the risk of developing fibromyalgia – these same genes are also associated with other functional somatic syndromes and major depressive disorder. The central symptom of fibromyalgia, namely widespread pain, appears to result from neuro-chemical imbalances including activation of inflammatory pathways in the brain which results in abnormalities in pain processing. The brains of people with fibromyalgia show functional and structural differences from those of people without fibromyalgia, but it is unclear whether the brain anomalies cause fibromyalgia symptoms or are the product of an unknown underlying common cause. Some research suggests that these brain anomalies may be the result of childhood stress, or prolonged or severe stress.

Fibromyalgia is recognized as a disorder by the US National Institutes of Health and the American College of Rheumatology. Fibromyalgia is described as a "central sensitization syndrome" caused by biological abnormalities in the nervous system which act to produce pain and cognitive impairments as well as psychological problems. Despite this, there is controversy as to the cause and nature of fibromyalgia, as well as how people affected are described by those in the medical community. Dr Frederick Wolfe, lead author of the 1990 paper that first defined the diagnostic guidelines for fibromyalgia, has stated he believes the causes of fibromyalgia "are controversial in a sense" and "there are many factors that produce these symptoms – some are psychological and some are physical and it does exist on a continuum."

Patients affected by Fibromyalgia

Fibromyalgia is estimated to affect 2–8% of the population, with a female to male incidence ratio that is somewhere between 7:1 and 9:1. The term "fibromyalgia" derives from New Latin, fibro-, meaning "fibrous tissues", Greek myo-, "muscle", and Greek algos, "pain"; thus the term literally means "muscle and connective tissue pain".

Management

As with many other medically unexplained syndromes, there is no universally accepted treatment or cure for fibromyalgia, and treatment typically consists of symptom management. Developments in the understanding of the pathophysiology of the disorder have led to improvements in treatment, which include prescription medication, behavioural intervention and exercise. Indeed, integrated treatment plans that incorporate medication, patient education, aerobic exercise and cognitive-behavioural therapy have been shown to be effective in alleviating pain and other fibromyalgia-related symptoms.

The Association of the Scientific Medical Societies in Germany, the European League against Rheumatism and the Canadian Pain Society currently publish guidelines for the diagnosis and management of FMS.

Medications

Health Canada and the US Food and Drug Administration (FDA) have approved pregabalin and duloxetine, for the management of fibromyalgia. The FDA also approves milnacipran, but the European Medicines Agency refused marketing authority.

Current market medicines

In 2007, Pfizers Lyrica became the first prescription medication approved for the treatment of fibromyalgia. Since then, two more drugs—Cymbalta and Savella—have joined its ranks, expanding the options to treat this painful chronic condition.

But because so much is still unknown about what causes fibromyalgia, there is no go-to treatment. In fact, typically only “35% to 40% of patients will respond well to any one of these three drugs,” says Daniel Clauw, MD, the director of the Chronic Pain and Fatigue Research Center at the University of Michigan, in Ann Arbor.

Research implementation

Research suggests that people with fibromyalgia may have extremely sensitive central nervous systems that perceive pain much more acutely than those with normal pain responses. There is also some evidence that people who suffer from fibromyalgia experience a range of sleep disturbances that may leave them more prone to pain. An even newer theory is that fibromyalgia may be related to compression of the cervical spinal cord, which can lead to sleep disruptions as well as widespread pain.

The three drugs currently approved for fibromyalgia are thought to ease the pain by acting on either the nerves or brain chemicals called neurotransmitters. Since these drugs do not have a high success rate, patients often use more than one or try several before finding one that works well with the least side effect

The 2012 Institute of Medicine Report about Pain in America reveals that more than 100 million Americans are in pain with an annual price tag more than 65 billion dollars.  The cost of fibromyalgia to both the individual and to society is extensive. A 2007 study found that 34% of fibromyalgia patients spend between $100- $1,000 per month above their insurance to see a healthcare professional. Prominent fibromyalgia researchers and specialists estimate the costs in the U.S. between $12-14 billion each year and accounts for a loss of 1-2% of the nation’s overall productivity.

The findings from the Pharmacor topic entitled Fibromyalgia reveal that, through 2020, an expanding drug-treated population and, to a lesser extent, the uptake of Forest Laboratories/Cypress Bioscience's Savella (milnacipran), Johnson & Johnson/Grünenthal’s Nucynta ER (tapentadol ER) and Pfizer’s controlled-release formulation of Lyrica (pregabalin CR), will help to offset sales lost to the generic erosion of Cymbalta and Lyrica. The fibromyalgia drug market will experience minimal growth over the next decade, increasing from nearly $1.6 billion in 2010 to less than $1.8 billion in 2020 in the United States, France, Germany, Italy, Spain, the United Kingdom and Japan.

In 2010, Cymbalta unseated Lyrica as the U.S. patient- and market-share leader. According to interviewed experts, Lyrica’s side effects, in particular its CNS side effects and weight gain side effect have significantly curbed the use of this agent in the fibromyalgia population.

The findings also reveal that the U.S. accounted for nearly 80 per cent of major-market fibromyalgia sales in 2010, and in 2020 the U.S. will account for two-thirds of worldwide sales for the indication. The majority of U.S. sales continue to be driven by the leading fibromyalgia-approved therapies—Cymbalta, Lyrica and Savella. In Europe and Japan, market expansion will be constrained by a dearth of approved therapies, lower diagnosis rates and less widespread recognition of the disorder.

Because fibromyalgia is a difficult condition to treat its symptoms vary widely from patient to patient and can include pain, depression, fatigue and sleep dysfunction, significant unmet need remains for effective treatments.
